New Patient Registration

If you wish to register with the practice, please request registration forms from reception. Once the forms are completed and returned (ideally with a form of identification), you will be invited for a New Patient Medical at our Surgery POD.

The practice covers Shotts and Newmains, including Dykehead, Stane, Torbothie, Springhill, Allanton, Hartwood, Bonkle and Cambusnethan.

We do not accept patients who reside in Harthill, Salsburgh, Cleland, Waterloo, Overtown, Bogside or Wishaw.

Please ensure that we have your up to date contact details e.g. mobile and home telephone number, email address.

Online Registration

You can now register with us ONLINE. All the information you need is below. Please take the time to read our leaflets and information as we do operate differently to other Practices in the area.

Just print the relevant form and once you have completed this, you can scan and send the forms to us at burnbrae.medicalpractice@lanarkshire.scot.nhs.uk, or hand them in to our Practice. If you have any issues, we have paper copies available in the Practice - just pop in and pick one up, fill it out and hand it back in once completed.

These fact sheets have been written to explain the role of UK health services, the National Health Service (NHS), to newly-arrived individuals seeking asylum. They cover issues such as the role of GPs, their function as gatekeepers to the health services, how to register and how to access emergency services.

Temporary Registration

If you are ill while away from home or if you are not registered with a GP Practice but require medical advice, you can receive temporary treatment from the local GP practice for up to 3 months, by registering as a temporary patient. 

This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.

To register as a temporary patient simply contact the local practice you wish to use. Practices do not have to accept you as a temporary patient. You cannot register as a temporary patient at a practice in the town or area where you are already registered.

The Practice has the right to remove patients from the Practice list under Schedule 1, Part 2, Section 10 of The National Health Service (Primary Medical Services Section 17C Agreement) (Scotland) Regulations 2018.

Patient Choice

If the patient informs the Practice they no longer wish to be part of the Practice, they can choose to register with another GP Practice in the area. The patient does not have to inform the Practice directly. The patient's new GP Practice will start this process.

GP Practice Choice

The Practice can remove a patient from the Practice list if they have 'reasonable grounds'. We will inform the patient in writing of our decision.

Violence and Aggression

If any patient commits an 'act of violence' against any member of our team, any member of staff carrying out their duties at our premises, or any member of the public within our premises, or behaves in such a way where anyone fears for their safety, we will request the patient be removed from the Practice list immediately.

Out of Area

If a patient has moved to a different address which is not within our Practice boundary, we will remove them from our Practice list. This is to protect our service and others should a home visit or medication delivery be required.
