If you have been invited to conduct a routine review, there are different ways to arrange the relevant tests/tasks. Please read the following information below carefully.

Please take time to complete your review as it lets us know if you require additional support.

Blood Tests and/or Foot Checks:

Urine Tests:

• Please collect a sample kit from our Reception and hand it back to the Practice before 2.30pm

Please also ensure you complete the Medlink review via the link in your invitation.

Once we have all this information, a member of our team will be in touch.
